The incident occurred while preparations were being made for a special effects sequence. The statement released read in part:
“During a filming session, a sudden and unexpected explosion occurred in a designated effects preparation area outside of the film set where James was experimenting on an unauthorized Pyrothechnic stimulation involving a gas cylinder alongside his two assistants.”
9. He later succumbed to injuries on the same day
Following the accident, Akaie was rushed to a hospital but sadly passed away on in the hospital due to the severity of his injuries. Two assistants who were present were reportedly injured and hospitalised. See statement below:
